What is the eligibility criteria to qualify for the Care Finder program?
There are some government eligibility requirements taken into consideration when someone wants to access in home care services.
Here’s the criteria:
- You need help with one or more everyday tasks
- You are aged 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people)
- OR you are aged 50 years or older (45 years or older for Aboriginal or Torres Strait Islander people) on a low income and homeless or at risk of being homeless
Reasons you may require intensive support:
- Isolation or no support person
- Communication barriers, including limited literacy skills
- Difficulty processing information to make decisions
- Resistance or hesitance to engage with aged care, institutions, or government for any reason
- Personal safety is at risk, or it may end up as a crisis situation
There may be other reasons not mentioned.
Better Place Australia can support you on the journey
If you have no-one to help you, one of our practitioners can support you through the process to determine if you qualify, including assessment through My Aged Care. If you do qualify, you’re ready to go and we can then support you to set up the services you need in your home.
Do you need a referral?
No. You can self-refer. A friend or family member can refer you. You can also be referred by your GP or other health and community services.
