What is the eligibility criteria to qualify for the Care Finder program?

To qualify for the program, you need to meet certain criteria (government eligibility requirements) to access in home care services.

  • You need help with one or more everyday tasks
  • You are aged 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people)
  • OR you are aged 50 years or older (45 years or older for Aboriginal or Torres Strait Islander people) on a low income and homeless or at risk of being homeless

Reasons you may require intensive support:

  • Isolation or no support person
  • Communication barriers, including limited literacy skills
  • Difficulty processing information to make decisions
  • Resistance or hesitance to engage with aged care, institutions, or government for any reason
  • Personal safety is at risk, or it may end up as a crisis situation

There may be other reasons not mentioned.

 

Better Place Australia can support you on the journey

If you are in a situation where you have no-one to support you, that’s where our practitioners step in. We can support you through the process to determine if you qualify, including assessment through My Aged Care. If you do qualify, you’re ready to go. Next step is helping you set up the services you need in your home.

 

Getting a referral

It’s easy to get a referral. You can self refer, or a GP or health service can you refer you. A friend or family member can also refer you.
